Elevate Care Niles, IL is an excellent facility for skilled nursing & rehabilation. I chose this facility for the care of my loved one and I was not disappointed. There are several positive things that I will mention. The facility is very clean, never smelled bad, like some places do. The staff were friendly and attentive. A very special note of recognition to Marcela Hernandez. This beautiful woman was phenomenal in every way. She was so attentive, professional, understanding, compassionate, and ALWAYS willing to go above and beyond the standard of what it means to be helpful and professional. I cannot thank her enough for all that she did to assist with any concerns that I or my loved one had. She was very communicative and relatable. Ms. Hernandez is a very hard working young lady, & is an asset to Elevate Care, and to anyone blessed to be acquainted with her. The facility is very fortunate to have her as an employee, she truly made the difference in, both my and my loved one's experience while there. I'd be amiss if I failed to mention the stellar physical therapy from Daroda, Mike, & occupational therapy, and speech therapy with Greta. Please excuse if I failed to mention any other therapists, you guys were amazing! My loved one struggled to walk upon admission and the team of therapists were instrumental in getting my loved one to walk via an excellent treatment plan with exercises & activities to enhance my loved ones health & well being. My loved one also mentioned CNA Sandy was so kind and helpful with showers, and other personal requests. Many thanks for all who were instrumental, & provided positivivity, & professional care for my loved one, kindness and professionalism never go unnoticed. Communication with staff was sometimes an issue but it is reasonable to understand as there are several residents and the staff is busy but attentive, caring, and willing to assist. Overall I am so pleased with Elevate Care Niles, IL, & I absolutely recommend it for skilled nursing & in-patient rehabilitation services.

My husband has been here doing rehabilitation services. The staff from the nurses,down to the therapists, are so friendly. My husband was at another rehabilitation center before Elevate Care. The other Rehabilitation Center was trying to say that my husband would be bed ridden. At Elevate Care, my husband has been working with the physical therapist and the occupational therapist, he's now able to walk with the assistance of a walker. He can use his wheelchair and get around also. He can use the bathroom on his own. He's working on doing stairs. He can shower by himself and get himself dressed now. What a difference a little motivation makes,by caring staff!

The care and attention my mother received during rehabilitation was great. All the staff from the nurses, the staff in the dinning room& activities, custodial, the therapists, to the social worker were amazing. I’m deeply appreciative of the care and time Ms Hernandez dedicated to myself & my sister who were overwhelmed with my mother’s condition. Her genuine attention to my mother put us at ease and we are forever grateful. Thank you 💜

It was a heart warming experience from the moment we met with Ms Hernandez to each and every person that helped my sister and myself with our mom’s care. It was and is an unforgettable experience, and was extremely appreciated to have each and every staff member there taking such tenderness with our mom as well as with us! My non-elderly dad recently spent several months at Elevate Care North Branch for dialysis and physical therapy following a stroke. While this was a very difficult time for him and for our family, the excellent staff at Elevate Care made sure we always felt welcome, well-informed and in the very best hands when it came to his care. I especially want to thank Norbert from Admissions for always being available to update us in the Polish language and Mike from Physical Therapy for going above and beyond in helping my dad get back on his feet. Thank you to all of Elevate Care's nurses, doctors and social workers for being so caring, attentive and understanding in this trying time. By the end of his stay, my dad surprised us by saying that he would be happy to stay at Elevate Care North Branch if he were not able to come home for whatever reason, such was the standard of his care.

Real nice place. Nurses very helpful and pleasant. The respiratory therapist was very informative and a nice person overall. I was impressed when I walked in. From the front desk receptionist to the nurse station staff I give them an A+. Your facility have great people working there.

My experience with Elevate North Branch was mostly great !!!
The reason I was sent there is insurance didn’t want to keep paying for acute therapy .
I was saddened.
But the therapy department at Elevate North Branch worked very hard with me and just like at the acute therapy, I made great progress !!!
Juliana, Julita, Both Mikes, Ray and my cheerleader kept me motivated on my successful days and when I failed .
People like Marcella,Amanda,Angie,DeeDee, Tiana, Natasha & others did a great job with keeping me well !!!
Aniya, Zee & Denecka and the entire Activity dept did a great job with keeping the spirits of residents up.
The new dietary director Felipe hit the ground running and has improved the meals greatly !!!
All that overshadowed any problems I had and the Lord blessed !!!
Still recovering but yet blessed !!!

My wife was scheduled to be there for 5 days. After 2 days of being there they called her by the wrong name and treated her for someone else. They then wanted us to sign paperwork that stated she received several treatments that she never received. This place has no idea what they are doing.
I would not send my dog there. We discharged after 2 days and because of that they would not answer any questions or help us in any way. We are reporting this to the State to have this place inspected. If you read all the reviews at all their branches they should be shut down. Please do not send anyone to any Elevate care especially the North branch location!
